Care Planning in NHS Oxfordshire and NHS Gloucestershire
Early pilot results in England indicate significant reductions in use of NHS services – up to 60% - by patients with a care plan. Spurred on by this, NHS Oxfordshire and NHS Gloucestershire are leading the way in adopting patient-centric care plans for all patients with a long term condition.
Furthermore, the PCTs have determined than an IT-supported approach to care planning is essential if the full benefits for patients and the NHS are to be derived.
What is it that IT adds to the care planning process?
• Pre-Preparation. The opportunity for patients to prepare for their consultation in advance using an interactive online service, meaning time savings for busy health teams.
• Support. Online care plan templates making it easy for health teams to set up a care plan during a care planning consultation.
• Storage. Care plans stored electronically, centrally and securely.
• Access. Care plans which are accessible, with patient permission, to a wide range of health and allied professionals who can help patients achieve their goals.
• Reporting. Analysis and auditing to meet Information Governance and commissioning needs.
• Communication. Emails and alerts to ensure that the care planning ball is not dropped either by patients themselves or by those supporting them.
Both PCTs have commissioned “MyCarePlan” from Intouch to provide this essential IT support and the Oxfordshire service will launch first in early March, with Gloucestershire a few weeks behind.
