A Care Plan software solution to support patients with any long term condition
Early research indicates substantial reductions in the use of NHS services – GP visits, A&E, outpatient visits and unscheduled hospital admissions – by patients who have a Care Plan.These are new style care plans which are developed in agreement with the patient and owned by the patient, focusing on what health and daily living improvements the patient wants to achieve – not the traditional clinician-led care plans for patients with complex conditions.
“MyCarePlan” is an IT system (also provided as a paper based booklet) which has been devised by Intouch with Health to help the NHS establish this new form of personalized care planning for all 15m patients with a long term condition. It is already being commissioned and is attracting the attention of an increasing number of SHAs and PCTs.
