My Care Plan - Helping patients to help themselves

MyCarePlan, a web-based system, slots into existing primary care procedures, eg a patient’s annual health check, and aims to reduce the  impact of care planning on practice time. Patients can participate through a range of media including online and by using a booklet. DiTV and other formats are planned.

MyCarePlan helps patients to consider and decide in advance of their consultation which personal health goals are most important to them and their care plan.  They are prompted with goals they may like to consider as well having the opportunity to state goals of their own. 

It gives them immediate self help advice and signposting to local resources, taking some pressure off primary care to supply this information AND stimulating some patients to set about tackling goals themselves.
