We have carried out extensive research with patients, professionals and management representatives with both Oxfordshire and Gloucestershire PCTs to ensure that My Care Plan meets users' requirements. These research sessions have thrown up many fascinating issues and challenges which Intouch and the PCTs are addressing very effectively…..
Issues and challenges
• Who is actually going to do the care planning consultations?
• How long will be available for these and how can system design cut down the time taken for busy health professionals?
• What should (and should not) be included in these new style, patient-centric care plans for all LTC patients?
• What information governance requirements need to be met?
• What are the arguments for and against integration of a care planning system with other existing medical systems?
Thorough evaluation will be taking place – including the impact on NHS service usage – and reports will appear in this website.
