“Info2you” information prescription system

Info2you has been created to help NHS Trusts comply with the Department of Health’s policy on Information Prescriptions. For example, the DH’s January 09 directive on Care Planning states that by 2010 every patient with a long term condition should be offered a care plan by their GP and that this should include an information prescription. Info2you is designed to make it very quick and easy (10 seconds) for a busy health professional to create and issue a personalised information prescription for a patient. The instructions on the prescription slip, which can be printed or emailed to the patient, can be in a variety of languages. Pathway-related, pre-set information prescriptions
Central to the system is the idea of pre-set information prescriptions. These are clusters of information items that are relevant to where the patient stands on their care pathway, eg “newly diagnosed” or “first hospital admission”. Instead of the busy doctor having to think what to prescribe and how to get hold of the information, this has all been pre-prepared. The doctor can add or delete items if they wish. The information prescribed can be national or local and there are many ways the patient can have their information dispensed to them:- by email
- online
- via a help desk
- via a call centre
- via a touch screen kiosk
- at a local library
