“Staywell” self care education programmes for reduced hospital admissions

The Staywell range of self-care education programmes is designed to help patients with long term conditions to improve their quality of life and to keep them out of crisis care. As part of a strategy to reduce unscheduled hospital admissions, Staywell represents an extremely affordable way to deliver self-management support to thousands of LTC patients in a PCT or Health Board catchment area.

Many long term conditions are addressed – COPD, diabetes, angina, hypertension, heart disease, and more – as well as related issues such as medication concordance and support for carers.

The programme audits the level of self-care knowledge a patient has, and whether any important check-ups or lifestyle advice have been missed. It then delivers relevant advice sheets and signposting to local services which can help to plug the gaps identified. Staywell produces valuable analysis helping to identify which patients have the least self care competence (and are therefore at most risk of needing emergency care) so that they can be given extra, preventive support.
