Care Planning for all 15m LTC Patients
The Department of Health has set a high priority on establishing care planning for all 15 million patients with a long term condition. It wants to see fast progress on this issue. This will probably mean introducing a simpler form of care planning than that which already exists for patients with severe illnesses and complex needs. It may be that this extra new form of care planning is focused on patients who are routinely dealt with in primary care and fits into the annual health check process.

Goal setting for patients
