Reducing Crisis Care amongst Older Patients
Illness and accidents amongst older people not only result in unscheduled hospital admissions, but often in other forms of crisis care, such as residence in a care home, and the loss of independence which often goes with this. These events are an enormous drain on health and social care budgets. Staywell 75 is an education programme designed to give older people good basic advice on keeping healthy and accident free, while at the same time identifying to health and social care workers which Staywell participants are most at risk of needing crisis care.
