Establishing Self Care Education for many more LTC patients
A key priority within the NHS is improving self care education. In busy primary care consultations there is often little time for educating patients in effective self management of their condition. Audits show that over 50% of patients with a long term condition will tend to have important gaps in their self care knowledge which may lead to an unscheduled hospital admission.
