Long Term Conditions covered by the Staywell Self Care Education Programme
The Staywell range of self care education programmes covers a wide and expanding range of long term conditions and related issues. The main conditions covered today are:- Diabetes
- COPD
- Hypertension
- Heart Failure
- Angina
- Atrial Fibrillation.
Stroke Prevention will be available soon, along with other conditions.
Medication Concordance, Pain management and Health of the Elderly
In addition to the conditions covered, the Staywell range also covers important related issues including:Medication Concordance
This is a major issue for the NHS with £billions lost annually through misuse and waste. Patients known to have problems with their medication – understanding it, taking it, storing it – can be introduced to this education programme.
Health of the Elderly
As well as providing self care education to help older citizens remain healthy and independent, this programme acts as an early stage health risk detector and helps identify people who may end up in crisis care if some early interventions are not made, such as falls prevention advice.
General Self Care Support
This is for patients and carers living with a long term condition and helps them deal with other challenges that face them such as transport problems, obtaining the benefits they are entitled to, problems at work as their condition worsens, carers needing respite, and many other issues.
